DPCH Day Program is a community access service aimed towards equipping persons served with fundamental enrichment skills, providing a positive and meaningful perspective on daily life.
Services are structured to improve or maintain the person served skills and ability to live as independently as possible in the community.
Services include training and supervision to an individual with self-care, task completion, communication skills, interpersonal skills, problem-solving skills, socialization, sensory/motor skills, mobility, community transportation skills, reduction/elimination of maladaptive behaviors, money management skills, and skills to maintain a household.
The program caters to each participant’s Individualized Service Plan, which outlines an individual’s strength, needs, and choice. Each individual follows a daily activity schedule for training in areas of Community Access.
These services are designed to assist the participant in acquiring, retaining, or improving self-help, socialization, and adaptive skills required for active participation and independent functioning outside the participant’s home or family home.
Community access is provided in a group setting up to 10 individuals. This service can take place in a day program building or in the community with a 1:6 staff to participant ratio.
